PERSONAL TRAINING SERVICE AGREEMENT

Tania Horton (Trainer) Responsibilities to you (Client):

✓ Tania Horton will design a personalised programme of exercise based on your needs and goals. 

✓ Each session is typically 30 minutes to 1 hour in duration.

✓ Tania Horton will provide guidance regarding proper exercise techniques.

✓Tania Horton will maintain a record of your progress and provide necessary feedback.

✓ Tania Horton will provide on-going evaluation and modification of the programme as necessary according to your progress, needs and goals.

✓ All information relating to you is confidential and will be treated in accordance with the Privacy & Information Act NZ.

Your Responsibilities as Tania Hortons Client:

✓ Sessions must be paid in full prior to the session.

✓ You are expected to discuss all health history information and any medical concerns with Tania Horton

✓ You will communicate any discomforts, pain or concerns experienced during or arising from a session.

In consideration of participation in a Personal Training Programme, I understand that I must purchase a single session or package of training sessions (block booking) and must read, agree to and sign this agreement.

I understand that the programme designed by the trainer is voluntary and I may be required to undergo a health and fitness assessment that will be fully explained to me. I agree to complete the medical history questionnaire/s accurately and completely including disclosure of any prescribed medications I am taking and any exercise or diet limitations I am aware of or have been informed of by my doctor and any changes that may occur.

I understand that I have the right to stop at any time during a session and that I should inform Tania Horton immediately of any symptoms such as fatigue, shortness of breath or chest discomfort.

I understand that exercise involves certain risks including, but not limited to, spinal injuries, heart attack, stroke, death and injuries to bones, joints or muscles. I agree to voluntary participation in the exercise programme and assume all risks.

I declare that I have read, understand and agree to the contents of this agreement in its entirety. I do hereby waive, release and discharge the trainer (Tania Horton The Aquatic PT) from any and all claims or liability for any present and future injuries or damages resulting or arising from my participation in any activities including but not limited to exercise, personal training or use of the equipment including any injuries and damages caused by the negligent act or omission of any of those persons or entities mentioned above.​

PROGRAMME OBJECTIVES:

PERSONAL TRAINING: I understand that my physical fitness programme is tailored to meet the goals and objectives agreed upon by my personal trainer and myself. GROUP TRAINING CLASSES: I understand that the physical fitness programme is designed to accommodate multiple individuals with varying goals and fitness levels.

LOCATION: Marlborough Lines Stadium 2000 on Kinross Street unless otherwise arranged.

DESCRIPTION OF THE EXERCISE PROGRAMME:

I understand that my exercise programme will involve participation in a number of types of fitness activities.

These activities will vary depending upon the objectives that my personal trainer and I establish, but will probably include:

1) Aerobic activities including, but not limited to, the use of treadmills, stationary bicycles, step machines, rowing machines and cross-trainers;

2) Muscular endurance and strength building exercises including, but

not limited to, the use of free weights, weight machines, bodyweight movements and exercise equipment;

3) In water activities including the use of but not limited buoyancy or drag equipment for resistance, resistance bands and frisbees. 4) Other activities selected by my personal trainer and agreed upon by me;

5) Selected physical fitness and body composition tests.

DESCRIPTION OF POTENTIAL BENEFITS

I understand that a regular exercise programme has been shown to have definite benefits to general health and well-being. I know that some of the benefits can include loss of weight, reduction of body fat, improvement of blood lipids, lowering of high blood pressure, improvement of cardiovascular function, reduction in the risk of heart disease, improved muscular strength and endurance, improved posture and

improved flexibility.

PARTICIPANT RESPONSIBILITIES

I understand that it is my responsibility to:

1) Fully disclose any health issues or medications that are relevant to participation in an exercise programme;

2) Cease exercise and report promptly any unusual feelings (e.g. chest discomfort, nausea, difficulty breathing, apparent injury) during the exercise

programme;

3) Clear my participation with my physician.

PARTICIPANT RESPONSIBILITIES

In agreeing to an exercise programme:

✓ I acknowledge that my participation is completely voluntary

✓ I understand the potential physical risks involved in the exercise programme and believe that the potential

benefits outweigh those risks

✓ I give consent to appropriate physical touching that may be necessary to ensure proper technique and body alignment

✓ I understand that health and fitness results may vary from person to person

✓ I have had a voice in approving the activities selected for my exercise programme

✓ I have been given opportunity to ask questions regarding any concerns I might have and have had those questions answered to my satisfaction

✓ I am in good physical condition, have no impairment which might prevent my participation in such activities and have been advised to consult with a physician prior to beginning this programme

✓ I have been advised to cease activity immediately if I experience unusual discomfort and feel the need to stop.
