APP BASED SERVICE AGREEMENT
PARTICIPANT RESPONSIBILITIES
I understand that it is my responsibility to:
1) Fully disclose any health issues or medications that are relevant to participation in an exercise programme
2) Cease exercise and report promptly any unusual feelings (e.g. chest discomfort, nausea, difficulty breathing, apparent injury) during the exercise programme
3) Clear my participation with my physician
In agreeing to this exercise programme:
*I acknowledge that my participation is completely voluntary
*I understand the potential physical risks involved in the exercise programme and believe that the potential benefits outweigh those risks
*I understand using my phone or other electronic equipment beside the pool is at my own risk
*I understand that health and fitness results may vary from person to person
*I state I am in good physical condition, have no impairment which might prevent my
participation in such activities and have been advised to consult with a physician prior to beginning this programme
*I have been advised to cease activity immediately if I experience unusual discomfort and feel the need to stop.
We strongly recommend against exercising alone, especially in water. In case of an
emergency, it’s crucial to have others around who can assist or call for help.
DESCRIPTION OF THE EXERCISE PROGRAMME:
I understand that my exercise programme will involve participation in a number of types of fitness activities. These activities will vary depending upon the objectives that my personal trainer and I establish, but will probably include:
1) Aerobic activities
2) Muscular endurance and strength building exercises
3) In water activities including the use of but not limited buoyancy or drag equipment,
resistance bands
4) Other activities selected by my personal trainer and agreed upon by me
5) Selected physical fitness and body composition tests.
Tania Horton will design all programmes
Each session is typically 30 minutes to 1 hour in duration.
Tania Horton will provide guidance regarding proper exercise techniques.
Tania Horton will maintain a record of your progress and provide necessary feedback.
All information relating to you is confidential and will be treated in accordance with the
Privacy & Information Act NZ. Your Responsibilities as Tania Horton's Client:
You are expected to discuss all health history information and any medical concerns with Tania Horton
You will communicate any discomforts, pain or concerns experienced during or arising from a session.
I understand that the programme designed by the trainer is voluntary and I may be required to undergo a health and fitness assessment that will be fully explained to me. I agree to complete the medical history questionnaire/s accurately and completely including disclosure of any prescribed medications I am taking and any exercise or diet limitations I am aware of or have been informed of by my doctor and any changes that may occur.
I understand that I have the right to stop at any time during a session and that I should
inform my medical provider immediately of any symptoms such as shortness of breath or chest discomfort.
I understand that exercise involves certain risks including, but not limited to, injuries to bones, joints, muscles, ligaments or tendons, spinal injuries, heart attack, stroke, death. I agree to voluntary participation in the exercise programme and assume all risks.
I understand that by downloading The Aquatic PT App I have read, understand and agree to the contents of this agreement in its entirety. I do hereby waive, release and discharge the trainer (Tania Horton The Aquatic PT) from any and all claims or liability for any present and future injuries or damages resulting or arising from my participation in any activities including but not limited to exercise, personal training or use of the equipment including any injuries and damages caused by the negligent act or omission of any of those persons or entities mentioned above.
